0165(2)(a) multiple plastic outlet covers throughout the building accessible to toddlers 0165(2)(d) plastic bags accessible to children in the bathroom changing area 0165(3)(a) Around the fire place there is ripped foam that is exposing the sharp edge of the fire place that is accessible to children; outside on the outer edge/corner of the building the siding is coming away from the side of the building and has sharp edges and is accessible to children; in the outside area bythe stairs their is a hook and nail that are protruding and accesible to children. 0165(3)(d) in the laundry/diaper changing room is the washer and dryer and the latch was missing being latched and 1 was broken to make both of these inaccessible to children 0165(5)(f) there was an extention cord in the front room being used to plug the TV into. 0165(3)(f) children cubbies missing being secured to the wall and pose a tipping hazard; the white corner cabinet, kitchen and book shelf in the back room are missing being secured to the wall and pose a tipping hazard 0165(3)(g) in the back child care room there were broken paper boxes with holes in them that were accessible to children. multiple shelves that hold materials throughout the building had the paint off of them in multiple areas, and there were a few shelves with actual holes in them all accessible to children; the shelves in the bathroom above the sink have flaking paint and is bubbling which can create splintering hazards to children and is accessible to the children. 0165(4)(c) In the outdoor area door by the stairs there is a light that is missing being coverd and is not shatter resistant
